I, ____________________, the undersigned applicant (or parent/legal guardian of applicant), declare myself to be at least 18 years of age, of sound mind and legally competent to enter into this agreement, as follows:
I agree to release Crest and Crown, Inc.,, together with its affiliates, parents, subsidiaries, officers, employees, contractors, and/or agents and managers (collectively, “Crest and Crown, Inc. ”) from all liability surrounding my participation in the Crest and Crown Pageant, including all other activities taking place before, during and after the pageant, including but not limited to any and all lost or damaged personal property items and any injury occurring to me before, during, or after the pageant.
I agree to indemnify and hold harmless the Crest and Crown Pageant from and against any claims, costs (including attorney’s fees and court costs), expenses, damages, liabilities, losses, or judgments arising out of or in connection with, any claim, demand, or indirect consequence relating to the Crest and Crown Pageant related to any negligence, nonperformance, breach of contract or other conduct of mine.
I agree to allow the Crest and Crown Pageant the sole and exclusive right and discretion to use my name, image, likeness and/or photographs of me, videos and/or DVDs containing my image, in any manner that the pageant so chooses, related to the Crest and Crown Pageant, for this year or subsequent years, without any prior written or verbal consent from me.
I agree to allow the Crest and Crown Pageant to, at its sole discretion, provide my name, email address, and/or title information to Pageant sponsors, contractors, and/or other Pageant affiliates, including but not limited to photographers and videographers.
Typing your full name in the electronic-signature field constitutes your signature and acceptance of this agreement.
